Appearing Before an Agent

What you need to bring when applying for a passport.

When appearing before the acceptance agent, you must have:

  1. Proof of Citizenship
    • A certified copy of your U.S. birth certificate issued by the Department of Vital Statistics, which must include:
      1. Applicant's full name
      2. Applicant’s date and place of birth
      3. Date the certificate was filed in the state registrar's office
      4. Registrar’s signature
      5. Parents' full names
      6. A raised, impressed, embossed, or multi-colored seal
    • -OR- a U.S. passport (may be expired)
    • -OR- a U.S. Certificate of Naturalization or Citizenship
    • -OR- a Consular Report of Birth Abroad
  2. One Passport Photo – taken within the past six months and showing your current appearance. Photo must:
    • Be color
    • Be 2" X 2"
    • Show a clear image of your face
    • Be taken against a plain white or off-white background
    • Be printed on quality photo paper
    • Do not wear glasses, photo-shop the image, or wear a uniform or camouflage.
    • Vending machine prints, selfies, full-length photographs, & school pictures are not acceptable.
  3. Current Identification - Valid driver's license or other acceptable picture ID. If your ID is out-of-state, less than one year old, or limited term, you’ll be asked to present two additional forms of secondary identification.
  4. Checks or money orders for payment - There will be two separate fees for each application. We do not accept credit or debit cards.
  5. Completed application - The form DS-11 is the only passport application processed in this office.

For All Minors Under Age 16:

  1. The minor child must appear in person.
  2. Applications for children younger than 16 require both parents' or legal guardians' consent.
  3. You must present proof of your parental relationship, even if your child already has a passport. Usually, this is your child's birth certificate.

For All Minors Age 16 to 17:

  1. The minor child must appear in person.
  2. For security reasons, parental consent must be shown.
  3. One parent must accompany the child, present identification, and co-sign the application if the child doesn't have a driver’s license of their own.
