What to do before your WIC appointment
- Call or email to make an appointment.
- Expect a reminder phone call from a WIC staff person the day before your appointment.
- Gather these documents to bring to the WIC office:
- Identification, such as:
- Oregon driver's license
- State ID card, a passport
- Work or school ID
- Oregon Health Plan medical ID card
- Current WIC ID card
- Proof of income, such as:
- Oregon Health Plan Healthcare.gov approval letter
- SNAP (food stamps) award letter*
- Temporary Assistance to Needy Families (TANF) Notice of Approval, or most recent Change Notice letter
- 1 month of most recent pay stubs
- Unemployment benefit stub
- Most recent W2 forms or income tax return
- Foster child/parent placement letter
- Signed letter from employer stating gross earnings
*We are unable to accept the Electronic Benefits Card (Oregon Trail Card) as proof.
- Proof of residence, such as:
- Oregon driver's license
- State ID card
- Current utility bill
- Bank statement
- Rent receipt
- Identification, such as:
If the appointment is for your child, also bring:
- Proof of identity, such as a birth certificate, an immunization record or an Oregon Health Plan medical ID card
- Their immunization record, if you have it
What to expect at your WIC appointment
At your appointment we'll find out if you're eligible and ask to see proof of income, identification and address. We'll also check your health.
- You and your child will be weighed and measured.
- Your child's growth and development will be discussed with you.
- You will receive a hemoglobin or iron screening.
- Your eating habits will be reviewed.
- Your medical/nutritional history will be taken.
- Together we'll review nutrition topics and set goals.
- You'll get an electronic benefits card to buy WIC-approved foods at your local grocery store.
